In submitting an Adult Beginner Enrolment Form, you are accepting the following terms and conditions.

Unsigned Enrolment Forms (paper copy) will not be accepted

Enrolment Forms
All Students must have a completed and signed enrolment form to participate in swimming lessons at the Paragon Swim Centre. If any changes occur then a new form will need to be completed. Please Note:one form Per Student

Payments
$25.00 Deposit required for booking. Deposits are only made refundable upon cancellation if 24 hours notice is given prior to the day of your booking. The remaining balance is to be paid prior to the beginning of the lesson. If the remaining balance is not paid then admission to the lesson will not be granted.

Payments can be made into the Paragon Swim Centre Pty Ltd bank account:- 
BSB: 105:135  Acct No: 6048 9040.

Lesson Package Conditions
The lesson package includes a 30 minute 1 on 1 session with an instructor and a 5 casual visit pass with a 3 month expiry so the student will be able to practice. The student may book in again to another session, but the price will remain the same as will the package deal. The 5 visit pass will be given to the student at the commencement of their lesson.

Unable to Attend Lesson
If the student is unable to attend the lesson 24 hours notice is required prior to the time of their lesson (eg. Lesson is at 6.30pm Tuesday then the centre needs to be notified before 6.30pm Monday), then the student will be booked into the next available date. To notify the centre please use the Paragon Swim Centre APP or email : paragonswim@gmail.com

If the lesson has been paid for and the student has failed to notify the centre the student will only receive the 5 visit pass and the lesson will be void.

Safety Issues
In the interests of safety students are not permitted to enter the water before the start of their lesson.  All internal doors are to be kept clear of all personal items at all times. Access behind the pool cover and the terracotta is not permitted to the public, this is a staff only area. 

Video / Photographs
Photographic devises include: cameras, video cameras, mobile phones with cameras, and any other device with the ability of capturing a still or moving image.

When using photographic devices, the activities caught in the image should be localised to your family members.

Permission is given for family and family friends to video or photograph students in lessons. If by accident the image of another student/patron is captured PERMISSION is NOT GIVEN to post image to social media networks and should be deleted immediately.

Permission is NOT given and the centre does not approve of tagging Staff Members of the Paragon Swim Centre via videos or photos on social media networks.

Management reserves the right to view any images taken. Refusal may result in the Police being called to view these images.

Use of photographic devices in the change rooms is extremely forbidden.

Technology in the Class Environment
To provide the best possible teaching and learning environment, instructors may use technology within the swimming lesson. The use of iPad's and teaching applications allows students to see immediately where their strengths and weaknesses lie withing their swimming technique.

By signing/submitting this form you agree to being videoed for teaching purposes only. These images will only be for evaluation and swimming educational purposes only and will not appear on any social media networks.

Pandemic Clause
In the event of a Global/Local Pandemic where State or Federal Government forces the closure of the swim centre (for example Covid-19) the swim centre will transfer any monies paid as credits towards another lesson. They will not be refunded they will be deducted from the following lessons fees.  Management will notify via Facebook post, App notification and information will be posted on the website.
